This kind of fear is something I've become very interested in lately.
I've found two books that I like very much, though so far I have only
skimmed them:

The first is for managers who wish to create a better environment so that
people in their employ are not afraid to say what's on their minds. The
second is for people who have been afraid of rocking the boat, and for
people who already speak up and need more skill in the way they present
and discuss their messages.
