The NO FAULT perspective in organizational change also works in individual
situations. I ALWAYS consider defects to be process-related and gather
information as to what was it that caused this defect to be possible.
If people can screw up, chanes are that they eventually will.

So defect free porcesses have to be built wherever the impact of these
inevitable screwups is large enough to justify the investment. Although
this is really TQM, there is an aspect that says "What can the organization
LEARN from this instance and what should we do about it?"

I believe this single question can lead to more insights and, if management
always ACT when the learnings occur, more truly L-O behaviour than all the
techniques will ever produce!
